Iodine I 131 ethiodized oil (commonly known as 131I-Lipiodol) is a radiopharmaceutical consisting of the radioactive isotope iodine-131 conjugated to ethiodized oil (Lipiodol), a contrast medium derived from poppyseed oil. It is designed for the targeted treatment of hepatocellular carcinoma (HCC) via intra-arterial administration. The ethiodized oil component acts as a selective carrier that is preferentially taken up and retained in the hypervascular microenvironment of liver tumors. Once localized, the iodine-131 isotope delivers high-energy beta radiation (and a small amount of gamma radiation) directly to the malignant cells. This internal radiotherapy causes lethal DNA damage and cell death while minimizing radiation exposure to healthy liver parenchyma. It has been used primarily as a palliative treatment for non-resectable liver cancer or as an adjuvant therapy following surgical resection to reduce the risk of tumor recurrence.
