Iodohippuric acid is a small molecule radiopharmaceutical primarily used as a renal imaging agent to assess effective renal plasma flow and kidney function. It is an analog of p-aminohippuric acid, labeled with radioactive iodine isotopes (such as I‑125 or I‑131), and is administered intravenously for renography. The compound is rapidly cleared from the bloodstream by tubular secretion in the kidneys, making it highly suitable for dynamic kidney imaging. In patients with normal renal function, approximately 85% of administered iodohippuric acid can be recovered in urine within 30 minutes after injection. Its mechanism of action relies on its filtration and active secretion by the renal tubules, allowing quantitative assessment of renal plasma flow and tubular function[1][3][7]. Historically, it has been considered a standard for measuring effective renal plasma flow but has largely been replaced by newer agents due to concerns about radiation exposure from certain isotopes[7][8].
