**iohexol + sulfobutylether-beta-cyclodextrin** is a combination formulation of the iodinated contrast agent iohexol and the excipient sulfobutylether-beta-cyclodextrin (SBECD). This formulation, notably under the investigational brand Captisol Enabled-iohexol, is designed to enhance the safety of iohexol in patients requiring vascular imaging. Iohexol functions as a non-ionic, water-soluble radiocontrast agent, allowing visualization of body structures during X-ray-based procedures by blocking X-ray transmission in iodine-containing tissues. SBECD is added to mitigate nephrotoxicity and reduce contrast-induced acute kidney injury (CI-AKI), especially in high-risk populations. Preclinical evidence demonstrates that SBECD-iohexol reduces renal and myocardial injury by interfering with early apoptotic processes in renal cells and preserving cardiomyocyte integrity during myocardial ischemia. The formulation is currently in phase II clinical trials for prevention of CI-AKI in patients undergoing coronary angiography[1][3].
