Iomab-ACT is an investigational antibody radiation conjugate (ARC) developed by Actinium Pharmaceuticals. It consists of apamistamab, a murine monoclonal antibody targeting CD45, conjugated to the radioisotope iodine-131. Iomab-ACT is designed for targeted conditioning prior to cell and gene therapies, including allogeneic bone marrow transplant (BMT), CAR-T cell therapy, and gene therapy. By specifically binding to CD45—a marker expressed on blood cancer cells and immune cells—Iomab-ACT delivers targeted radiotherapy to deplete immune cells (lymphodepletion) or achieve reduced intensity conditioning. This approach aims to replace traditional non-targeted chemotherapy or total body irradiation regimens with a more selective method that minimizes toxicity while enabling successful engraftment of donor or genetically engineered autologous stem cells[1][2][3][4][5]. The drug is currently being evaluated in clinical trials for use in patients with sickle cell disease undergoing BMT and may be further studied as a conditioning agent before gene therapies[2][3].
