Overview

Iopanoic acid is an iodine-containing small molecule radiocontrast agent formerly used in cholecystography. It acts as a potent inhibitor of thyroid hormone release from the thyroid gland and inhibits the peripheral conversion of thyroxine (T4) to triiodothyronine (T3) by blocking thyroxine 5'-deiodinase enzymes. This mechanism makes it effective for rapid control of hyperthyroidism, particularly in severe cases or when standard antithyroid therapies are ineffective or contraindicated. Iopanoic acid has also been used as adjunctive therapy before thyroidectomy and in neonatal thyrotoxicosis. Its use for treating thyrotoxicosis has been discontinued in several countries, including the United States[1][2][4][5].
