IOV-4001 is a genetically modified autologous tumor-infiltrating lymphocyte (TIL) therapy developed for the treatment of unresectable or metastatic melanoma and advanced non-small cell lung cancer (NSCLC). It uses TALEN gene-editing technology to inactivate the gene encoding programmed cell death protein 1 (PD-1) in TILs, thereby removing an immune checkpoint that normally inhibits T-cell activity. By knocking out PD-1, IOV-4001 aims to enhance the antitumor activity of infused T cells by preventing their suppression within the tumor microenvironment. The therapy involves isolating and expanding a patient’s own TILs from resected tumor tissue, editing them ex vivo to knock out PD-1, and then infusing them back into the patient following a preconditioning chemotherapy regimen. This approach combines adoptive cell transfer with intrinsic immune checkpoint inhibition within a single product[2][4][5][7].
