Iparomlimab + regorafenib is an investigational combination therapy consisting of iparomlimab, a humanized IgG4 monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, and regorafenib, an oral multi-kinase inhibitor. Iparomlimab functions as a checkpoint inhibitor, blocking the interaction between PD-1 and its ligands (PD-L1 and PD-L2) to reactivate T-cell mediated anti-tumor immunity. Regorafenib inhibits a broad range of protein kinases involved in tumor angiogenesis (VEGFR1-3, TIE2), oncogenesis (KIT, RET, RAF-1, BRAF), and the tumor microenvironment (PDGFR, FGFR). This combination is being explored for its potential synergistic effects, where regorafenib-mediated modulation of the tumor microenvironment and angiogenesis may enhance the efficacy of PD-1 blockade. Clinical development is notably being led by Peking Union Medical College Hospital for the treatment of advanced solid tumors, including anal squamous cell carcinoma, peritoneal mesothelioma, and peritoneal metastatic cancer.
