Overview

Iparomlimab + tuvonralimab + lenvatinib + EP/EC is an investigational combination regimen being evaluated for the first-line treatment of unresectable or metastatic extrapulmonary neuroendocrine carcinoma (EP-NEC). The regimen's immunotherapy backbone, QL1706 (also known as Qibeian), is a bifunctional combination of two monoclonal antibodies: iparomlimab (targeting PD-1) and tuvonralimab (targeting CTLA-4) in a fixed 2:1 ratio. This is combined with lenvatinib, a multi-kinase inhibitor targeting VEGFR and FGFR, and standard platinum-etoposide chemotherapy (etoposide plus either cisplatin or carboplatin). The combination aims to enhance anti-tumor activity by simultaneously blocking multiple immune checkpoints, inhibiting angiogenesis and tumor cell proliferation, and inducing cytotoxic cell death.
