Overview

Iparomlimab + tuvoraleimab + bevacizumab + nab-paclitaxel + carboplatin is an investigational five-drug combination regimen being evaluated for the treatment of advanced acral and mucosal melanoma. The regimen combines dual immune checkpoint inhibition with anti-angiogenic therapy and cytotoxic chemotherapy. **Iparomlimab** is a monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, while **tuvoraleimab** (also known as tuvonralimab) targets the cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). These two antibodies are often administered as a fixed-dose co-formulation known as **QL1706** (brand name Qibeian), developed by Qilu Pharmaceutical. **Bevacizumab** is a monoclonal antibody that inhibits vascular endothelial growth factor (VEGF), and the chemotherapy backbone consists of **nab-paclitaxel** (an albumin-bound nanoparticle formulation of paclitaxel) and **carboplatin** (a platinum-based alkylating agent). This intensive combination is designed to address the low response rates of acral and mucosal melanomas to standard PD-1 monotherapy by enhancing T-cell activation and modifying the tumor microenvironment.
