This is a combination therapy consisting of three agents: - **Ipatasertib** is an oral small molecule inhibitor of AKT (protein kinase B), which plays a key role in the PI3K/AKT signaling pathway involved in cell proliferation and survival. - **Pertuzumab** and **trastuzumab** are monoclonal antibodies that target different epitopes on the human epidermal growth factor receptor 2 (HER2). Both block HER2-mediated signaling but through distinct mechanisms; their combination provides dual HER2 blockade. This regimen is being investigated for use in patients with unresectable locally advanced or metastatic HER2-positive breast cancer harboring PIK3CA mutations, particularly as maintenance therapy after first-line treatment with taxane plus HP (pertuzumab/trastuzumab)[1][2][3][4]. The rationale for this combination is to simultaneously inhibit both the PI3K/AKT pathway and HER2-driven signaling to overcome resistance mechanisms and improve antitumor efficacy.
