Ipilimumab + fotemustine is a combination therapy used primarily in the treatment of advanced or metastatic melanoma, including patients with brain metastases. Ipilimumab is a fully human monoclonal antibody that targets cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4), thereby enhancing antitumor immune responses by blocking an inhibitory signal to T cells and promoting their activation and proliferation. Fotemustine is an alkylating agent of the nitrosourea class that can cross the blood-brain barrier and exerts its effect by causing DNA damage in tumor cells. The combination has demonstrated clinical activity in phase 2 trials for metastatic melanoma, including those with brain metastases[1][3][5][6]. Ipilimumab was developed by Bristol Myers Squibb.
