Ipilimumab + nivolumab + relatlimab is an investigational combination of three immune checkpoint inhibitors, each a monoclonal antibody targeting a distinct immune regulatory pathway. Ipilimumab targets cytotoxic T-lymphocyte–associated antigen 4 (CTLA-4), nivolumab targets programmed cell death protein 1 (PD-1), and relatlimab targets lymphocyte activation gene-3 (LAG-3). By simultaneously blocking these inhibitory pathways, the combination aims to enhance T-cell activation and antitumor immune responses more robustly than dual or single-agent checkpoint blockade. This triple regimen is being studied for advanced or unresectable melanoma, with the goal of improving efficacy while managing toxicity compared to existing doublet regimens[3].
