Overview

**ipratropium + RPL554** is an investigational combination of two inhaled bronchodilators being evaluated for the treatment of Chronic Obstructive Pulmonary Disease (COPD). Ipratropium is a short-acting muscarinic antagonist (anticholinergic) that works by inhibiting muscarinic acetylcholine receptors in airway smooth muscle, leading to bronchodilation. RPL554 is a novel inhaled dual phosphodiesterase 3 and 4 (PDE3/4) inhibitor with both bronchodilator and anti-inflammatory effects. Mechanistically, RPL554 increases intracellular cAMP and cGMP levels in airway smooth muscle and inflammatory cells, producing relaxation (bronchodilation) and reducing inflammation. Clinical studies show that the combination of ipratropium and RPL554 produces greater bronchodilation and reduction in lung hyperinflation compared to either agent alone, with good safety and tolerability. The combination is being developed primarily for COPD, especially in patients who continue to experience symptoms despite standard bronchodilators[1][2][3][4][5].
