QUARTET LDQT (also known as GMRx4) is an investigational, ultra-low-dose quadruple-combination therapy developed for the treatment of hypertension. It consists of four established antihypertensive agents—irbesartan (an angiotensin II receptor blocker), amlodipine (a calcium channel blocker), indapamide (a thiazide-like diuretic), and bisoprolol (a beta-1 selective blocker)—each provided at one-quarter of their standard therapeutic doses (37.5 mg, 1.25 mg, 0.625 mg, and 2.5 mg, respectively). The rationale behind this "quadpill" approach is to achieve additive blood pressure lowering efficacy by targeting multiple physiological pathways simultaneously while minimizing dose-dependent side effects associated with standard-dose monotherapy. The combination was pioneered by The George Institute for Global Health and has been evaluated in the Phase 3 QUARTET trial and the Phase 2 QUARTET USA trial, demonstrating superior blood pressure control compared to standard-of-care monotherapy.
