irbesartan + hydrochlorothiazide + amlodipine is a triple combination antihypertensive therapy consisting of an angiotensin II receptor blocker (ARB), a thiazide diuretic, and a dihydropyridine calcium channel blocker (CCB). This specific regimen is being evaluated by the Third Military Medical University in Phase 3 clinical trials (e.g., NCT03541200) for the treatment of uncontrolled hypertension, often as a comparator to interventional procedures like adrenal artery ablation (AAA). Irbesartan acts by selectively blocking the binding of angiotensin II to the AT1 receptor, thereby inhibiting vasoconstriction and aldosterone release. Hydrochlorothiazide promotes diuresis and reduces blood volume by inhibiting the sodium-chloride symporter in the distal convoluted tubule. Amlodipine induces peripheral vasodilation by inhibiting the influx of calcium ions into vascular smooth muscle cells. Together, these three agents provide a synergistic effect to lower blood pressure in patients who are not adequately controlled on dual therapy or single agents.
