Overview

This combination therapy regimen consists of low-dose irinotecan administered as a radiosensitizer in conjunction with CyberKnife stereotactic body radiotherapy (SBRT). Developed and investigated by Wake Forest University Health Sciences, the treatment is primarily indicated for patients with colorectal cancer who present with limited liver metastases. Irinotecan, a camptothecin derivative, functions by inhibiting DNA topoisomerase I, which stabilizes the cleavable complex and prevents DNA ligation, ultimately leading to lethal double-strand breaks during SBRT. The CyberKnife system provides image-guided, robotic delivery of high-dose radiation with sub-millimeter precision, allowing for the ablation of liver lesions while minimizing damage to adjacent healthy hepatic tissue.
