Overview

Irinotecan + selenomethionine is an investigational combination therapy consisting of the chemotherapeutic agent irinotecan and the selenium-containing amino acid selenomethionine. Irinotecan is a topoisomerase I inhibitor that interferes with DNA replication in cancer cells, leading to cell death. Selenomethionine is an organic form of selenium thought to have antioxidant properties and potential protective effects against chemotherapy-induced toxicity. The rationale for combining these agents is to enhance antitumor efficacy while potentially reducing irinotecan-associated toxicities, particularly gastrointestinal side effects. Phase I studies have evaluated dosing regimens and pharmacokinetics, showing that high doses of selenomethionine can be safely combined with standard-dose irinotecan, though no major protection against toxicity was established; some clinical benefit was observed in heavily pretreated patients[1][2][7].
