Irinotecan + thalidomide is an investigational combination therapy consisting of two small molecule drugs, irinotecan and thalidomide. Irinotecan is a topoisomerase I inhibitor that interferes with DNA replication in cancer cells, leading to cell death. Thalidomide acts primarily as an antiangiogenic agent by inhibiting the formation of new blood vessels that supply tumors, and also has immunomodulatory properties. This combination has been studied in clinical trials for various advanced solid tumors, including recurrent glioblastoma multiforme (GBM), metastatic colorectal cancer, and recurrent central nervous system (CNS) tumors. The rationale for combining these agents is to enhance antitumor efficacy by targeting both tumor cell proliferation (irinotecan) and tumor vasculature (thalidomide)[1][2][3][4][5].
