Drug intelligence / Profile preview
Irinotecan liposomal (nal-IRI) is a formulation of the chemotherapy drug irinotecan encapsulated within a lipid bilayer vesicle (liposome). This design prolongs plasma levels of both irinotecan and its active metabolite SN-38, increasing antitumor activity compared to conventional formulations. Irinotecan and SN-38 inhibit topoisomerase I, an enzyme responsible for producing reversible single-strand breaks in DNA during replication. By stabilizing the topoisomerase I-DNA complex and preventing re-ligation of DNA strands, they induce double-strand DNA breaks leading to cell death. The primary indication for nal-IRI is metastatic pancreatic ductal adenocarcinoma previously treated with gemcitabine-based therapy; it is also being studied or used in other cancers[5][9].
