Drug intelligence / Profile preview
Irinotecan liposomal is a nanoliposomal (nanosomal) formulation of irinotecan, encapsulated within a lipid bilayer vesicle (liposome) to modify the pharmacokinetics and tissue distribution of the drug. Irinotecan is a topoisomerase 1 inhibitor with antineoplastic activity, acting by binding to the topoisomerase 1-DNA complex and preventing DNA re-ligation, ultimately causing double-strand DNA breaks and cell death. Its active metabolite, SN-38, is a more potent inhibitor of topoisomerase 1, contributing significantly to its cytotoxic effects. The liposomal (nanosomal) formulation extends the plasma half-life and exposure of irinotecan and SN-38, leading to increased antitumor activity compared to conventional irinotecan. Irinotecan liposomal (Onivyde) is FDA-approved for use in combination with fluorouracil and leucovorin, with or without oxaliplatin, for the treatment of metastatic pancreatic adenocarcinoma, especially following progression on gemcitabine-based therapy. It is under evaluation in other malignancies[1][2][5][7][10].
