Irinotecan liposomes is a nanoparticle-encapsulated formulation of irinotecan, a cytotoxic small molecule that acts as a topoisomerase I inhibitor. This formulation is designed to enhance the therapeutic index of irinotecan by prolonging circulation time and increasing drug accumulation within the tumor microenvironment. Zhejiang Cancer Hospital is investigating this agent in a Phase I trial for locally advanced rectal cancer, where it is administered weekly in combination with capecitabine-based neoadjuvant chemoradiation. The treatment strategy utilizes UGT1A1 genotyping to guide dose escalation, aiming to optimize efficacy while minimizing the risk of dose-limiting toxicities such as neutropenia and diarrhea.
