irinotecan sucrosofate

Irinotecan sucrosofate, also known by the brand name Onivyde, is a nanoliposomal formulation of the semisynthetic camptothecin analogue irinotecan. In this formulation, irinotecan is encapsulated within liposomes as an irinotecan sucrose octasulfate (sucrosulfate) salt. It functions as a prodrug, converted to its active metabolite SN-38, which is a topoisomerase I inhibitor. SN-38 inhibits topoisomerase I activity by stabilizing the cleavable complex between topoisomerase I and DNA, thereby preventing DNA religation and leading to lethal double-strand DNA breaks during replication. This mechanism results in antineoplastic activity. The liposomal encapsulation improves pharmacokinetics, extends circulation time, and enhances drug delivery to tumor sites, potentially leading to fewer side effects and improved efficacy compared to non-liposomal irinotecan. It is developed for various cancers, including pancreatic and ovarian cancers.
