Irofulven + oxaliplatin is a combination of two small molecule chemotherapeutic agents used experimentally for the treatment of advanced solid tumors, including hepatocellular carcinoma, endometrial cancer, gastric cancer, liver cancer, pancreatic cancer, and other solid tumors. **Irofulven** is a semisynthetic derivative of illudin S (from the mushroom *Omphalotus illudens*), acting as a novel DNA binding agent that induces DNA single-strand breaks, leading to cell cycle arrest (primarily in S phase) and apoptosis, independent of p53 and multidrug resistance status. **Oxaliplatin** is a platinum-based chemotherapeutic that forms DNA crosslinks, inhibiting DNA replication and transcription, leading to cell death. When combined, the drugs display synergistic or additive effects in inhibiting cancer cell proliferation and inducing apoptosis across a variety of cancer cell lines. Preclinical and early-phase clinical studies have reported encouraging antitumor activity and manageable toxicity profiles for the combination, particularly in hepatocellular carcinoma and other solid tumors.
