Iron isomaltoside, also known as ferric derisomaltose, is an intravenous iron replacement therapy consisting of a complex of ferric iron and isomaltoside 1000. It is designed to treat iron deficiency and iron deficiency anemia by providing a controlled release of bioavailable iron to iron-binding proteins, such as ferritin and transferrin, while minimizing the risk of free iron toxicity. This formulation's stability allows for the administration of high single doses in a short period, which is particularly advantageous in clinical settings like postpartum anemia where rapid restoration of iron stores is necessary. The University Medical Centre Ljubljana has investigated its efficacy in treating postpartum anemia through a Phase 4 randomized trial (NCT03957057), comparing it to other intravenous and oral iron formulations to assess hematological parameters and patient-reported outcomes such as fatigue and depression.
