Iron polymaltose (iron(III)-hydroxide polymaltose complex) is an oral and parenteral iron preparation used to treat and prevent iron deficiency and iron deficiency anemia. It consists of a macromolecular complex of trivalent iron (Fe3+) hydroxide with the carrier polysaccharide polymaltose. The drug is absorbed in the small intestine via a controlled active mechanism, minimizing unbound free iron in circulation. Once absorbed, the iron binds to transferrin for transport to the bone marrow where it is incorporated into hemoglobin for red blood cell production. Iron not immediately used is stored as ferritin or haemosiderin in tissues such as the liver. Unlike traditional ferrous salts, this preparation releases iron gradually and does not generate free radicals, reducing oxidative stress and gastrointestinal side effects[1][3][5][6].
