Overview

Irradiated donor lymphocytes (IDL) are an investigational cellular immunotherapy consisting of lymphocytes (predominantly T-cells) harvested from an allogeneic donor and treated with ionizing or ultraviolet radiation. The irradiation process is designed to induce DNA damage that prevents the clonal expansion and proliferation of the donor cells within the recipient, thereby significantly reducing the risk of Graft-Versus-Host Disease (GVHD). Despite the inability to proliferate, these cells remain metabolically active and retain short-term cytotoxic capabilities. IDLs are utilized in various clinical contexts, including the facilitation of engraftment in hematopoietic stem cell transplantation (HSCT) by suppressing residual host immunity, the neutralization of anti-donor antibodies in HLA-incompatible transplants, and the induction of a graft-versus-tumor (GVT) effect in hematologic malignancies and solid tumors like renal cell carcinoma.
