IRS1 siRNA is a liver-targeted small interfering RNA (siRNA) therapeutic being developed by insitro for the treatment of metabolic dysfunction-associated liver disease (MASLD). Discovered through AI/ML-driven genetic analysis of UK Biobank data, the drug specifically targets insulin receptor substrate 1 (IRS1) in hepatocytes. By suppressing hepatic IRS1 expression, the siRNA reduces de novo lipogenesis (DNL) and hepatic triglyceride accumulation. Preclinical studies in human hepatocytes, diet-induced obesity (DIO) mice, and cynomolgus monkeys have demonstrated that a single dose can significantly reduce hepatic IRS1 levels and improve NAS scores without adversely affecting systemic glucose homeostasis.
