iSAP-0909 is a novel, self-assembling near-infrared (NIR) fluorescent contrast agent designed for intraoperative tumor delineation and image-guided surgery, particularly in bladder cancer. Developed by Isap (Beijing) Biotechnology, the agent consists of a peptide conjugated to a cyanine dye that specifically targets the X-linked inhibitor of apoptosis protein (XIAP). Upon binding to XIAP, downstream caspase enzymes are activated, which cleave the probe and trigger the in situ self-assembly of the dye into beta-sheet-enriched nanofibers. This mechanism leads to significantly enhanced accumulation and prolonged retention within tumor cells compared to conventional agents like hexyl aminolevulinate (HAL). Preclinical studies have demonstrated high sensitivity and specificity in human bladder cancer specimens and orthotopic models. The FDA has cleared an Investigational New Drug (IND) application for iSAP-0909, and clinical evaluation is ongoing through investigator-initiated trials.
