Overview

Isatuximab + bortezomib + lenalidomide + dexamethasone (Isa-VRd) is a quadruplet regimen used for the treatment of multiple myeloma. This combination therapy includes: - **Isatuximab**: A humanized monoclonal antibody that targets a specific extracellular epitope of CD38 receptor, which is widely expressed on plasma cells. It kills myeloma cells through multiple mechanisms including antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), complement-dependent cytotoxicity (CDC), and direct induction of apoptosis. - **Bortezomib**: A proteasome inhibitor - **Lenalidomide**: An immunomodulatory drug - **Dexamethasone**: A corticosteroid The combination has shown significant efficacy in clinical trials, particularly in newly diagnosed multiple myeloma patients who are ineligible for autologous stem cell transplantation. In the IMROZ phase 3 study, Isa-VRd demonstrated a 40% reduction in the risk of disease progression or death compared to VRd alone, with deep and sustained responses.
