ISB 1342 is a first-in-class bispecific monoclonal antibody designed to treat relapsed or refractory multiple myeloma. It simultaneously targets CD38 on tumor cells and CD3ε on T cells using the BEAT (Bispecific Engagement by Antibodies based on the TCR) platform. The drug features a high-affinity Fab arm for binding to a unique epitope of CD38 (distinct from daratumumab) and a detuned scFv domain for binding to CD3ε, which helps mitigate cytokine release syndrome risk. By bridging T cells and myeloma cells, ISB 1342 redirects cytotoxic T lymphocytes to kill malignant plasma cells regardless of their sensitivity to prior anti-CD38 therapies like daratumumab. Preclinical studies show potent activity against multiple myeloma cell lines with varying levels of CD38 expression and in models resistant to daratumumab[4][5][6]. The drug is being developed by Glenmark Pharmaceuticals and Ichnos Sciences.
