Iscalimab is a fully human monoclonal antibody that targets CD40 antigen, a costimulatory receptor expressed on antigen-presenting cells such as B cells, dendritic cells, and macrophages. By blocking the CD40-CD40L (CD154) pathway, iscalimab inhibits T cell and B cell activation, thereby suppressing immune responses implicated in autoimmune diseases and transplant rejection. Developed by Novartis (with early involvement from XOMA), it has been investigated for multiple indications including type 1 diabetes mellitus, myasthenia gravis, lupus nephritis, hidradenitis suppurativa, Graves' disease, systemic lupus erythematosus (SLE), renal transplant rejection, liver transplant rejection, rheumatoid arthritis (no current development), and Sjögren's syndrome (development discontinued). Iscalimab can be administered intravenously or subcutaneously[3][4][6][7][8].
