ISIS 325568 is a second-generation antisense oligonucleotide drug developed to target the glucagon receptor (GCGR). Its primary mechanism of action is the inhibition of GCGR mRNA expression, leading to reduced hepatic and adipose tissue glucagon receptor abundance. This results in decreased hepatic glucose production, which can help improve blood glucose control in patients with type 2 diabetes. Additionally, ISIS 325568 has been shown in preclinical studies to increase circulating levels of glucagon-like peptide-1 (GLP-1), potentially enhancing pancreatic function and insulin secretion. The drug was investigated as a novel therapeutic approach for type 2 diabetes mellitus, aiming for disease-modifying effects without causing hypoglycemia or significant gastrointestinal side effects. Clinical development included Phase 1 trials assessing safety, pharmacokinetics, and pharmacodynamics in healthy volunteers.
