Overview

ISIS 449884 is a 2'-O-methoxyethyl (2'-MOE) antisense oligonucleotide that targets the glucagon receptor (GCGR). By binding to the mRNA of the glucagon receptor in liver and adipose tissue, it reduces GCGR expression and thereby inhibits glucagon signaling. This leads to reduced hepatic glucose output and lower blood glucose levels. The drug has been studied as both monotherapy and as an add-on to metformin in patients with type 2 diabetes mellitus (T2DM), showing significant reductions in HbA1c compared to placebo without increased risk of hypoglycemia. It represents a novel mechanism among antidiabetic agents by directly targeting glucagon action at the receptor level[1][5][7].
