Overview
Islatravir + ulonivirine is a once-weekly oral combination antiretroviral regimen in clinical development for the treatment of HIV-1 infection. **Islatravir (MK-8591)** is a nucleoside reverse transcriptase translocation inhibitor (NRTTI) that blocks HIV reverse transcriptase by inhibiting translocation on the primer:template and causing chain termination via its unique 4′-ethynyl and 3′-hydroxyl moieties. **Ulonivirine (MK-8507)** is a next-generation non-nucleoside reverse transcriptase inhibitor (NNRTI) designed for weekly dosing, retaining activity against viruses resistant to older NNRTIs, with a resistance profile similar to doravirine. Both drugs aim to simplify HIV therapy by reducing dosing frequency and pill burden, with current phase 2b trials in virologically suppressed adults evaluating safety, tolerability, and efficacy. The combination is being developed by Merck/MSD[1][3][4][5][6].
