Overview

ISM5059 is a peripherally restricted small molecule inhibitor of the NLRP3 (NOD-like receptor protein 3) inflammasome, developed by Insilico Medicine using its AI-powered Pharma.AI platform. Nominated as a preclinical candidate in February 2026, ISM5059 is engineered to inhibit NLRP3 conformational changes and oligomerization while remaining outside the central nervous system (CNS). This peripheral restriction distinguishes it from other NLRP3 inhibitors like ISM8969, which are brain-penetrant. The drug is intended for the treatment of various inflammatory and metabolic conditions, including obesity, diabetes, hyperlipidemia, and cardiovascular diseases. Preclinical studies have shown that ISM5059 dose-dependently inhibits the pro-inflammatory cytokine IL-1β and maintains an excellent safety profile without CNS-related toxicity.
