ISM5939 is a novel, orally available small molecule inhibitor of ecto-nucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), developed by Insilico Medicine using generative AI platforms. ENPP1 is an enzyme involved in nucleotide metabolism and purinergic signaling, influencing immune responses and cancer progression. Overexpression of ENPP1 is associated with metastasis and poor prognosis in several cancers, including breast, ovarian, and lung cancer. By inhibiting ENPP1, ISM5939 preserves extracellular cyclic GMP-AMP (cGAMP) levels, thereby activating the cGAS-STING pathway to enhance anti-tumor immunity through increased immune cell infiltration and activity within the tumor microenvironment. Preclinical studies have shown robust anti-tumor efficacy both as monotherapy and in combination with immune checkpoint inhibitors such as anti-PD-L1 or anti-PD-1 therapies. The drug has demonstrated favorable safety profiles in preclinical models. IND clearance was granted by the FDA for clinical trials targeting advanced solid tumors[1][2][3][4][5][6].
