ISM8207 is an orally administered, small molecule inhibitor targeting glutaminyl-peptide cyclotransferase-like protein (QPCTL). Developed using generative AI-driven drug discovery by Insilico Medicine in collaboration with Fosun Pharma, it is designed as a novel antineoplastic agent for the treatment of advanced solid tumors and B-cell lymphoma. Mechanistically, ISM8207 inhibits QPCTL, which disrupts the CD47-SIRPα "don't eat me" signal on tumor cells—thereby enhancing immune-mediated tumor cell clearance. Preclinical studies demonstrated that ISM8207 reduces SIRPα binding to CD47 and synergizes with anti-PD-1 therapy to promote anti-tumor immunity by increasing CD8+ T-cell infiltration into tumors. The drug has shown promising pharmacokinetics and safety profiles in preclinical models and is currently being evaluated in Phase I clinical trials for both solid tumors (including metastatic disease) and relapsed/refractory B-cell lymphoma[1][2][3][4][5][7].
