ISM9528 is a preclinical, orally available, brain-penetrant, non-opioid small molecule analgesic developed by Insilico Medicine for the treatment of chronic and acute pain, including neuropathic and postsurgical pain. Identified using the PandaOmics AI platform and optimized via the Chemistry42 generative chemistry platform, the compound acts on a novel, undisclosed biological target (referred to as 'Target Z') that was previously unassociated with pain management. In preclinical models, ISM9528 has demonstrated superior efficacy compared to standard-of-care therapies like morphine and pregabalin, while maintaining a favorable safety profile and excellent blood-brain barrier permeability. Insilico Medicine nominated ISM9528 as its 31st preclinical candidate in 2024, with plans to initiate clinical trials in 2027.
