Isobrill is a brand of isotretinoin marketed by VTS Life Sciences, available in 10mg and 20mg oral tablet formulations. Isotretinoin is a systemic retinoid and a derivative of Vitamin A (13-cis-retinoic acid) primarily indicated for the treatment of severe, recalcitrant nodular acne that has not responded to conventional therapies, such as systemic antibiotics. Its therapeutic mechanism involves the activation of nuclear retinoic acid receptors (RARs), which leads to a significant reduction in the size and sebum output of sebaceous glands, inhibition of follicular keratinization, and a decrease in the inflammatory response. By altering the skin microenvironment, it also indirectly reduces the population of *Cutibacterium acnes*. Due to its high efficacy and potential for serious adverse effects, including teratogenicity, its administration is typically subject to strict medical supervision.
