Overview

**Isoconazole** is an imidazole antifungal used primarily for the topical treatment of superficial skin and vaginal fungal infections, such as vaginal candidiasis and dermatomycoses[1][2][3][4][7][8]. It demonstrates broad-spectrum activity against dermatophytes, pathogenic yeasts, filamentous fungi, some gram-positive bacteria, and trichomonads[1][7]. Isoconazole acts by inhibiting the biosynthesis of ergosterol, a vital component of fungal cell membranes, leading to increased membrane permeability and cell death. Additional mechanisms include damage to the fungal cell membrane causing rapid ATP depletion and inhibition of tubulin polymerization[1][3][7]. Isoconazole was originally synthesized by Janssen Pharmaceutica and has been marketed in various countries (not the US), sometimes in combination with corticosteroids like diflucortolone. Limited systemic absorption occurs with topical or vaginal use, and efficacy is similar to clotrimazole in clinical trials[1][2][3].
