Overview

Isoflurophate (diisopropyl fluorophosphate) is an organophosphorus compound and irreversible cholinesterase inhibitor used primarily as ocular drops in the treatment of chronic open-angle glaucoma and other eye conditions such as accommodative esotropia. It acts by covalently binding to acetylcholinesterase and cholinesterase enzymes inactivating them and preventing the breakdown of acetylcholine. This leads to increased levels of acetylcholine at cholinergic synapses resulting in parasympathomimetic effects such as miosis (pupil constriction) and reduced intraocular pressure. Isoflurophate has also been used before or after certain eye surgeries to manage intraoperative or postoperative increases in intraocular pressure. Its use is limited due to its high toxicity; exposure can cause severe systemic symptoms if not handled properly[1][4][7][8].
