Overview

Isolated pelvic perfusion (IPP) is a locoregional treatment modality developed and studied at Gustave Roussy Cancer Campus for patients with non-resectable, recurrent gynecologic or digestive pelvic cancers. The procedure involves the surgical or percutaneous isolation of the pelvic blood circulation, often utilizing a pneumatic anti-shock garment (PASG) to prevent systemic leakage. High doses of tumor necrosis factor-alpha (TNF-α), a biologic cytokine that targets tumor vasculature, and melphalan, a small molecule alkylating agent that causes DNA damage, are delivered directly into the isolated pelvic circuit. This technique allows for drug concentrations significantly higher than those achievable through systemic administration while minimizing systemic side effects.
