Isomyosamine is a synthetic small molecule alkaloid derived from tobacco plant alkaloids, developed as an oral metabolic and immunomodulator. It acts primarily by modulating pro-inflammatory cytokines, especially tumor necrosis factor alpha (TNF‑α), but also interleukins such as IL‑6 and IL‑17A. Isomyosamine selectively inhibits overactive TNF‑α signaling without causing broad immunosuppression, distinguishing it from traditional TNF inhibitors that are associated with increased infection risk. The drug can cross the blood-brain barrier and has shown potential in preclinical studies to extend lifespan and reduce inflammation-driven muscle loss (sarcopenia/frailty) after hip or femur fracture surgery. Clinical development includes Phase 2 trials for sarcopenia/frailty in elderly patients post-fracture, with additional early-stage studies for autoimmune diseases such as rheumatoid arthritis and Hashimoto disease[1][2][4][5][7][10].
