Overview

Isoprenaline + phenylephrine is a fixed-dose combination of two sympathomimetic agents used primarily as an inhaled bronchodilator and in research models. Isoprenaline (also known as isoproterenol) acts as a non-selective beta-adrenergic receptor agonist, stimulating both beta-1 and beta-2 adrenergic receptors to produce bronchodilation, increased heart rate, and cardiac output. Phenylephrine acts predominantly as an alpha-1 adrenergic receptor agonist, causing vasoconstriction and increasing peripheral vascular resistance. The combination has been used clinically in pressurized inhalers (e.g., Medihaler-Duo) for reversible airway obstruction such as asthma but has largely been superseded by more selective agents due to side effect profiles. In preclinical research, the combination has also been used to induce heart failure models in animals by simultaneously activating both alpha-adrenergic and beta-adrenergic pathways[1][3][4][8].
