Overview

Isosorbide diesters + hydrocortisone is a combination therapy investigated for the treatment of atopic dermatitis. The isosorbide diester component, developed by Sytheon, consists of isosorbide di-(linoleate/oleate) (IDL) and isosorbide dicaprylate (IDC), which are fatty acid esters derived from sunflower and coconut oils. These compounds function by strengthening the epidermal barrier, enhancing skin hydration, and regulating keratinocyte differentiation. Additionally, they suppress several pro-inflammatory mediators, including IL-1β, TSLP, and transient receptor potential channels (TRPV3 and TRPA1). Hydrocortisone, a low-potency corticosteroid, is included to manage breakthrough flares and reduce overall skin inflammation. Clinical trials in pediatric and adult populations have demonstrated that this combination can reduce pruritus, improve disease severity scores, and decrease dependence on topical corticosteroids.
