Overview

Isosulfan blue is a triarylmethane diagnostic dye used for lymphatic mapping and sentinel lymph node (SLN) identification, particularly in breast cancer and melanoma. Following subcutaneous or intraparenchymal injection, the dye binds to interstitial albumin and is selectively transported via the lymphatic system, staining lymphatic vessels and nodes a distinct blue color. This visualization enables surgeons to identify the primary draining nodes for biopsy, facilitating accurate cancer staging while reducing the morbidity associated with complete lymph node dissection. While effective, its use is associated with a risk of hypersensitivity reactions, including life-threatening anaphylaxis in approximately 1% of patients, and it can cause factitious oxygen desaturation readings on pulse oximetry.
