Isotonitazene (ISO24) is a synthetic opioid belonging to the nitazene class of compounds. It was originally developed by Swiss researchers in 1957 as part of efforts to find new painkillers but was never marketed due to its extreme potency and high risk for addiction. Isotonitazene is structurally distinct from other synthetic opioids like fentanyl and belongs specifically to the chemical class known as nitrobenzimidazoles. It acts as a potent agonist at the mu-opioid receptor, with potency estimated at over 500 times that of morphine and about five times that of fentanyl. Isotonitazene has no approved medical use and has emerged in recent years on the illicit drug market, often found mixed into other drugs or pressed into counterfeit pills resembling prescription opioids[5][7]. Its use has been associated with numerous overdose deaths.
