Overview
Isotretinoin + silymarin is a combination therapy being investigated by Tanta University for the treatment of moderate to severe acne vulgaris. Isotretinoin is a systemic retinoid that addresses all four major factors in acne pathogenesis: sebum production, follicular hyperkeratinization, *Cutibacterium acnes* colonization, and inflammation. Silymarin, a standardized extract from milk thistle (*Silybum marianum*), is included for its antioxidant, anti-inflammatory, and hepatoprotective properties. The combination aims to enhance therapeutic efficacy while potentially mitigating oxidative stress and inflammatory markers such as IGF-1, SAA1, and malondialdehyde (MDA) associated with acne. This regimen is currently being evaluated in a Phase 4 clinical trial to compare its efficacy against monotherapy with either agent.
