ISPM21 is an investigational inhaled liposomal formulation of **GS-441524**, developed by InspirMed for COVID-19 prophylaxis and treatment. The product uses a **liposomal drug-delivery platform** to encapsulate the small-molecule antiviral GS-441524 and deliver it directly to the lungs by inhalation, with the goal of increasing pulmonary exposure while limiting systemic burden. GS-441524 is the principal circulating metabolite of remdesivir and acts as an antiviral inhibitor of **SARS-CoV-2 replication** through intracellular conversion to an active nucleoside triphosphate that inhibits the viral RNA-dependent RNA polymerase. Preclinical pharmacokinetic data described for ISPM21 indicate prolonged lung residence and markedly higher lung concentrations than intravenously administered GS-441524 solution, and the formulation is positioned as a way to avoid the cyclodextrin excipient used in remdesivir formulations.
