Isunakinra is a recombinant fusion protein engineered to inhibit interleukin-1 (IL-1) signaling by acting as an antagonist at the interleukin-1 receptor type 1 (IL-1R1). It contains domains from both IL-1β and the IL-1 receptor antagonist (anakinra), resulting in higher affinity for IL-1R1 than anakinra itself. Isunakinra was originally developed as a topical therapy for inflammatory diseases of the ocular surface, such as dry eye disease and allergic conjunctivitis, but its development for these indications has been discontinued. The drug is now being repositioned and developed primarily for oncology indications, including solid tumors and adenocarcinoma, where it is administered subcutaneously. Its mechanism involves blocking both IL-1α and IL-1β signaling through the IL-1 receptor, thereby reducing inflammation that contributes to tumor immune suppression and progression[3][8][9].
